If you DO NOT fill out any of your paperwork prior to your appointment you will need to reschedule.
If you show up more than 15 minutes late to your appointment you will be charged a $50 no-show/no-call fee. If you cancel or change your appointment less than 24 hours prior to your appointemnt you will be charged $50 fee.
